Thanks! I found that as well after I posted the request. I've been in touch with the publisher and am about to order the journal. Just in case anyone else is interested, here is how to order one. It is costly, but somehow I think it is a good one to have in a library. the price is in Singapore dollars with is about $80 U.S. This is the note I received:
